There is a book titled The Perfect Storm. I've not read it. Basically the phrase means that all wheather elements come togehter at the same time to exert the maximum destruction that an otherwise regular storm would not have been able to. For example, Sandy came not only with strong winds and heavy rain, a strong cold front from the north west met it in the New England area, as well as a full moon, which added couple of feet to the high tides. Thus the storm came at a perfect time to do its damage.
