To my understanding, "heartache" refers to emotional sadness while"chest pain" refers to physical illness.

For example:

Michelle felt heartache after she heard the sad news.

Tom suddenly got a severe chest pain while working in the office. His colleague had to rush him to the hospital.

Well, just my two cents.
