Ann Cook is not the only authority who doesn't distinguish the vowel sounds in the word small and that in hot.
I read most of the reviews on Amazon on Cook's book "American Accent Training". And the ones who don't give 4 or more stars mostly come with an English mother tongue, for what they are looking for is only the American Accent, pronunciation wise.
Despite the imperfection, I still highly value this book for its effectiveness in teaching us (especially Asians) the right intonation.

Yes, M-W uses the same symble for schwa and /&/, but
