My nephew will come to the USA in June to go to a local high school in our area. Information that I got from SingleOldCat was absolutely correct: check your local court and get the documents to apply for legal guardianship. If you need to add her to your insurance, you absolutely need the court documents. However, the minor must be present for such application.

Legal guardianship was not needed for visa application for my nephew's case.
