请教：瘫痪病人咳不出痰，常肺部发炎发烧，怎么护理？

家有老人脑梗瘫痪在床。吞咽功能失调，不能咳痰，都是咽下去。肺部老是发炎，低烧。现在住院，打抗生素消炎。有什么办法能把痰吸出来吗？因为整天躺着不会动，没食欲，加上吞咽不好，吃的很少，没有力气拉大便。有什么可以帮助缓解便秘的非处方药吗？在美国能买到的。
