For secondary progressive MS patients, relapses still happen but are relatively infrequent. The main feature is that complete remissions are absent. In other words, the patient progressively gets worse over time even between acute relapses. This deteriorating course can be very obvious and is different from a normal aging process.
Currently MS cannot be cured. DMTs have been shown to somewhat reduce the relapse frequency and slow down the disability progression. The effect is modest but real.
