The 2.7x0.4x1.4cm concavity thing is probably not a fibroid because it is unlikely a mass(fibroid) has one dimension of 2.7 CM and another dimension of 0.4 CM, unless it is a perdunculated submucosal fibroid within the endometrial cavity -- and then it should just say a solid mass. It could however represent a hydrosalpinx, now you are mentioning the exam is done for infertility (sorry if I am wrong). It could also represent synechia which could prevent from implanting. WHat did the radiologist say in the IMPRESSION?

But this ultrasound exam cannot determine patency of the tubes. You need HSG exam (hystrosalpinogram).
