急问脑中风问题: MDGG please enter again

Hi MDGG,

I am the son of the patient, thank you very much for your insightful information.

1. My mom was treated with tPA 2.5 hours after the initial stroke, I belive it is this medicine which converted the ischemic stroke to hemorrhagic stroke. The doctor told me that there is 6% for this risk and it is regardless of the age, the health conditions of the patient, I made the decision to let them to use it. The initial response of my mom to this medicine is good, she could move her left side a little bit a few hours after the IV tPA. But about 12 hours later, she started vomitting and get into coma. I read an article written my some spaniand saying that the c-Fn level in the patient's blood can be used to indentify if tPA can cuase hemorrhagic stroke(if c-Fn is greater than 3.6 mg/mm, it will cause hemorrhagic stroke). Probably this is not an established theory, have you heard of this? Also the doctor told the half-life period of the medicine is short, how can it took 12 hours for the hemorrhagic stroke happen?

2. Right now, they put my mom into coma using propofol(25mg/ml/min) and she is also intubated, there is still CSF coming out the pipe from her brain(it is about a few ml/hour). The doctor told me that for normal person, the CSF would join the blood circulation through some vessel, but for my mom, the vessel is blocked, and they have to wait for the blockage to be disolved. Also they told me that the blood and swollens in the brain has to be absorbed before the patient can be waken, and this may take weeks or even months, is this true? And I don't understand why there is another blockage to the CSF vessel, I thought the original clot which caused the initial stroke was in the artery and it was supposed to be removed by the tPA.

3. My mom still moves her right side a lot even under the sedation medicine and she is responding to some stimulus, does this mean she can wake up OK? What is the possibility of death upon the wakeup? And what determines if the patient can survive after the wakeup?
