Very common. Most common reason is because of too much bowel gas. Ultrasound is performed by ultrasound technologist, so it is very much operator dependent. Some techs even say at times "it doesn't feel like it that I can find the overay". It is not like CT/MRI/X-ray film that everything is on the film and it is the job of radiologist to identify the disease. Plus, if transvaginal ultrasound is not performed, the chances of seeing the overies are even lower.
