I would for sure do a fellowship such as pulmonary, hem-onc, GI, neurology, rheumo, or cardiology. But try to go to a big-name program. That will take you to a bigger group which has its own specialists -- no worry about referals since all patients are within your group.

I never regrated. My primary care friends on the other hand are not so happy. They really worry about patient resource so they have to advertise a lot. Even if they get patients coming to their door, the money is pitty (even plummer charges $80/hr). They are doing shitty stuff everyday, wife does coding, etc.
