OPT and H1b urgent help!!!

I applied for opt 2 month ago, and I checked it online and see it was approved, but never received the physical card. I called UICIS, and they asked me to re-file I765 form, does that means I should pay again, and include everything original documents? or just the form?
BTW, as I have started my work in a company, maybe I should apply for H1b now instead of opt?
