If your answers to both questions are NO, then what are you worried/frustrated about? He made mistakes, and he tried to cover his behind. As long as he's not blaming on you, you don't need to be concerned. But rather use that as an opportunity to help him. If you can help him reduce such mistakes, and/or fix those problems, you will gain his trust and support.
Now, if he's trying to shift the blame on you. Explaining to him won't help. You just need to make extra effort to bring more people into the process of problem solving, so everyone knows who fixed the problem and likely who caused the issue. Ccing emails is a very simply but powerful tool.
