Most of the top schools now (I shall say top 30 or even 50), researchwise, do not have real tenure track--you have money, you stay; you are out of money, you are out, does not matter if you are assistant, associate or full professor...they essentially "eat what they kill" (usu. it's used to describe lawyer)..
It's a tough life...but, it's a tough life in general ... be it doctors or lawyers...
