sick tenant refuse to leave after lease expiration

we are relocating back to where we were from. Rented out our single house to a single mom. 3 year lease is about to expire in two months. Tenants are citing health issues and refuse to move out at the end of the lease. We are willing to give her one more month to accomodate her situation. She is not lying as she has had a few surgeries. She works in law firm as well.

What is our best option? Will greatly appreciate advice.
