Former 401K - Rollover to IRA or current employer 401K?

I plan to move old 401K out of the old company, which I worked there 8 years ago... They started to charge fees a couple of years ago, which I just found out...:(

Which option is better -- Rollover to IRA or consolidate with my current 401K...

I plan to open an ira/roth ira account, which company is better? Fedelity, Vangaurd or others...
