大小地主请帮忙: how to renew lease

I found this renew form :http://www.thelpa.com/free/renew.pdf
but only see the place for tenant to sign. don't see any place for landlord to sign. So wondering if this is the right /legal form I should use to renew lease? Thank you all for the help!
