U.S. doesnt. Contractors or part-time employees usually have to pay 100% of the medical - a lot more than the group insurance employer plans.
Yeah, lots of jobs out-sourced to overseas, not just IT - such as the customer call-center jobs are sent to overseas too.
Basically, anything that can be done with remote communication is a candidate for outsourcing.
Canada has medical provided by government (OHIP)
