I am not sure if I understand you. How did CIS approve it if they never received your application? It seems to me that you were trying to withdraw your application. OPT is nothing much but a work authorization. I am not aware that there is any application form you need to complete in order to withdraw your application. Probably your school official just wrote a letter to CIS explaining that you have not graduated yet. To qualify for OPT you must graduate. I personally believe you will be fine. It is your school's mistake. I would suggest that you keep all the record, including completed application, CIS receipt, and your school official's letter, in case that you will have to prove your innocense in the future.
