I think there's more to the story LZ didn't tell everyone.

LZ claim "he charged me some service that he never did". Without medical billing knowledge, you can't tell what the doctor was billing for. If someone with this knowledge, she should already know what to do.
Unless LZ knew the doctor would bill more at the begining with and agree with for whatever reason.

Charge insurance company but without perform service is medical fraud. Can serve jail time! 99% of the doctors will not do that.
