Documents showing the number of shares of stock purchased as of date of separation (8/5/2022)
Documents showing what funds if any, under the name of You, were held at the time by the company for the purchase of additional shares of stock.
Document showing the date and number of shares of stock disbursed to You during the employment period.

Statements listing all benefits to which You is entitled, typically issued annually, or
Statements regarding one particular benefit that is issued on a periodic basis.
Stocks activities in company transaction: Finisar merged with IIVI on September 24, 2019
Finisar’s stockholders will receive $15.6 per share in cash and 0.2218xshares of IIVI common stock, valued at $10.4 per share. The transaction values Finisar at $26.00 per share.
