Actually the form was asked by the doctor office that states that the doctor's office reviewed the benefie with you and I agree to pay...

I got the bill from the doctor's office for the co-pay ($25) each time whenever the doctor office sent the visit code as Specialist visit although the visits were scheduled as the regular routine visits. I do not have any special issues but just the blood sugar issue during pregancy. It's been under control with my diet plan and excercise. My OB doctor just ask me every time during the regular visit, the same as "Do you feel baby moving"... those kinds of the general questions.
