Who make the hiring decision? Both or mainly by hiring manager?

Does that mean I already passed the hiring manager? If I get some wired questions (In TV show 'are you smarter than a fifth grader', some questions are basic and should-know, but can't get out of your head), what should I do? Thanks again.
