每个月premium 多少？ 包括公司+员工交的

不可能比这个低。这个family plan 一年2万多刀（公司+员工一起）

 

PPO:

In network

 $2,500 individual $4,500 family (Includes deductible, copays and coinsurance)

Out of network

$5,000 individual $9,000 family (Includes deductible, copays and coinsurance)
