1. use rapid dry yeast, not just dry yeast ( I think this is the key)
2. use bread flour only, not all purpose (2nd key)
3. add 1/2 to 1 teaspoon extra yeast more than the recipe
try the above 3.
4. one more thing you can try if you are around when the bread is making. Take the dough out after the last punch down (right before the last rise), then let it rise in 2 bread pan (let it rise for the same time as your bread machine), than bake for 20min or so.
My bread has always been a hit at home. Try all the tip above should get you a good basic bread, you can then vary a little bit a time to see if you still get good bread. Also use the bread machine basic recipe as your base.
