to make a start up floppy:

98: control panel - addremove programs starup disk
xp: format a floppy with create dos starup disk option.

You should have two ide buses. try other one.

add a raid card and hook your drive there.

do u have spare hard drive? try a know good hard drive on your machine. do you have spare pc try your hard drive on other pc. if everything is ok you got check your bios.
