老土， two questions about background and references check

1. 申请职位时通常要填两个表。 Job application form and reference form. For reference checking form， I know 谁说俺好就用谁。 For job application form where one needs to provide direct supervisor information, should/could I still use my direct supervisor if I don't have a good relationship with him? HR told me that my manage couldn't provide any comments on me (good or bad) except for my employment duration and salary.

2. My job applications seems to get some problems in background/reference check step. Where could I find a paid service to figure out who said what on me causing the problem. I remember someone recommended to use the service in my situation.
