I had a performance feedback talk with my boss

just to see if there is anything going on behind my back. I can tell I really caught him cold. He started off by saying my job performance was good, hitting all milestones etc. seemed to avoid teh key issue. So I practically cut him off and asked direct question: how did I do with communication. Then he went on and on about style and came back to the integrity issue (someone else's work, unfortunately I was the messenger and had to report it). Looks like they will dump all that garbage on me in my year-end performance review. My question is if I am leaving before year-end rating is completed, is this review going into HR record? If this review will be on record, how should I protect myself now? So far, I ahve not yet talked with HR about this, thinking that at least they would not accuse me of any wrong doing. I was too naive. any suggestions?
