Advice for self assessment

It is time for annual review, but we were asked to turn in self assessment first. I am confident to gave all 5 (perfect) for myself,but I am not so sure for communication. There is no problem to communicate with boss and team members but I am not a native speaker, should I give 5 for my communication?
Is there any guidance for self evaluation?
