Background check and Contract job

I worked as a contract job before. Now I am filling out an application
form for a new employer's background check.

The question is which supervisor should I fill in? Apparently, the
employer name should be the consulting firm. But what about the supervisor
and the phone number? Should I put the name of the manager in the
hosting company, or the recruiter in the consulting firm? Thanks a lot.
