 Below is the email their reply to me.

With regards to your enquiry. You each need your own visa application form along with the other documents stated on the form. Payment for each visa is CASH OR MONEY ORDER $55.00. Processing for the visa is 48 hours and the information for the Miami Office is stated on the form as well.
Attached hereto is a visa application along with the requirements for obtaining the visa. Please note that to expedite your request you must submit the completed application and supporting documents to the Bahamas Consular Office responsible for the State in which you reside.  

 

Please note that all applicants must be physically in the USA at the time the visa is presented.  If they are not, the visa will be denied.
