what shall I do with my 401K when changing jobs?

My old company have 401K for 13 monthes, less $5000.

My new company doesn't have 401K, (profit sharing, instead).

I don't have IRA, or ROTH IRA.

What is the income required for IRA, or ROTH IRA?

How to open a account? At Bank?
