Check with your co-workers first.

Since they want to give you a cognition evaluation, they should have some proof to back it up. If your illness is known to your co-workers, you should check with them about your daily behavior. If they confirm that you do have cognitive problems, then you do not have too much choices. If you don't, talk to a lawyer about it.

The point is that chronic illness can affect you in many ways. It could affect your memory, reasoning and judgement etc. So you need make sure you are doing OK before you start fighting.
