Slightly up MCV, could be macrocytic.  But is your hemoglobin level normal?  If it is normal, there is no worry about MCV.  Common macrocytic anemia results from B12 or folate deficiency, and etc.

These two are urine study.  If it is asymptomatic, no need to take antibiotics.

Guideline is a little complex.  Old one is lower than 7% but someone said lower than 6.5%.  The risk is always higher when A1C is > 4.5%.

It is true that microalbuminuria is a finding of early diabetic nephropathy.  However you only have one number here.  You will also need to have urine randon creatinine level with the same urine sample, so a calculation can be make to normalized albumin level to the amount of urine creatinine level.  Second urine after first void in the morning is the most accurate one.
