Vit D,1,25 OH2,Total: is the active form of vitamin D. It was formed in kidney by an enzyme called 1-alpha hydroxylase to add an extra 1-OH at 25-OH Vit D.
However, the measurement of Vit D,1,25 OH2 is often not accurate and variable. The total body storage of vitamin D is best estimated by 25-OH vitamin D level and the unit is ng/ml (30-100).
If 25-OH vit D is normal but 1,25 vit D is very low, the main problem could be due to kidney's inability to convert, which ofter happens at chronic kidney disease at or beyond Stage III (GFR usually lower than 60, mostly lower than 45).
