If the fever persists, you can take your child to hospital. Most of the time, reason for fever is difficult to find. If a child comes to pediatric ER, doctors will do tests to see if there is bacterial infection in blood, urine, and spinal fluids. If all are ruled out, then it is most likely to be viral. There is not much you can do for viral infection, except for lowering the fever, by tylenol and ice pack. If the fever persists, then go to ER again.
