请问,plastic surgery (去面部黑痣)的手术费用一般保险COVER 吗?

听说,如果不是可能发生癌变的,保险是按美容来算,不给COVER的.
是真的吗? 不切除,活检,怎么知道其性质?
打电话去plastic surgery office询问,得到答案very confusing.
