请各位帮忙看看这个乳房全切的病理报告

4月份正常乳房年检。6月初被要求做一侧乳房的core biopsy, 6月底在非常接近的位置（1.2CM）被要求做第二次core biopsy 。这两个位置被初步诊断为DCIS:stage 0, grade 2。手术医生说可以先选择局部切。但自己选择全切+Sentinel node biopsy.两周前做了该侧乳房的全切和3 sentinel node biopsy. 手术病理报告发现乳头上也有DCIS 并确诊为 invasive ductal carcinoma。请各位帮忙看看这个病理报告:

Final Diagnosis

1. invasive ductal carcinoma, no specific subtype

2.the greatest linear dimension of the tumor 0.4cm

3.Nottingham grade – 2

a. tubule formation – 3

b. nuclear grade – 3

c. mitotic rate – 1 (5 mitoses per 10 high power fields, high power field diameter 0.52mm)

d. overall Nottingham score 7/9

4. no lymphovascular invasion

5. all surgical margins are clear of invasive carcinoma and ductal carcinoma in situ

6. high-grade solid DCIS is present adjacent to the invasive tumor

a. the greatest linear dimension of DCIS in a single slide 0.6CM

b. high grade solid, cribriform, and micropapillary DCIS is present in the lower outer quadrant and the upper outer quadrant

c. small focus of high-grade solid DCIS in the nipple.

7. no involvement of skin by invasive carcinoma

8. negative for microcalcifications

9. background non-tumorous tissue shows fibrocystic change, small foci of usual ductal hyperplasia, focal fibroadenomatous change and numerous fat necroses

10. three lymph nodes are present in the axillary tail, all negative fr malignancy

11. status of tissue biomarkers:

a. estrogen receptors – positive (allred score 7)

b. progesterone receptors – positive (allred score 8)

c. Her-2/neu – negative (1+, weak membranous staining in approximately 5% of cells)

12. pTNM tumor stage – pT1a N0

手术医生解释说虽然比活检严重，可我的选择是正确的，所以还算乐观。正在等下一步的看肿瘤医生的治疗通知。45岁，没有生育过。现在心里很紧张。是不是很严重，可能会有什么样的后期治疗呢？以上的病理报告对下一步的治疗都有什么含义？请各位帮忙看看，万分感谢。在线等。。。。。。
