Do not BLAME him.

 

Next time he does something wrong or forgets something, say to him: I'm sorry, that's my fault..., then explain to him why it's your fault, with all the sincerity on your face and in your tone that you can muster, and no matter how ridiculous it may sound to you.

Keep doing it, and see if he changes.
