At the end of June to middle of July, but it might be a long odds usually.

If he still wanted to apply for MD PhD,  he might do it before significant improvements are achieved, for example, a manuscript being published in the well-respected journal as the 1st author in hands. 

He might also deeply rethink about his interviews to learn something, for example, conversations with interviewers , especially the faculty with PhD or MD PhD, regarding his research and beyond; comfortable and positive interactions with interviewers who have distinct backgrounds, and so on. 

In general, if an applicant is not able to secure one A after 3 IIs, the most likely reason is the performance of the applicant during the entire time for interviews. 

He might have to retake MCAT if he doesn't apply for medical school in the cycle of 2023-2024. However, rushing to apply for medical schools might not be a good idea, especially as a reapplicant, building up a solid and highly competitive application is.
