Dear [Mr. or Mrs. Superintendent's Name],

I am the mother of YOUR DAUGHTER'S NAME who is a third grader at SCHOOL NAME 1.  I write to you to request a transfer to SCHOOL NAME 2 to ensure safety of YOUR DAUGHTER'S FIRST NAME.  YOUR DAUGHTER'S FIRST NAME has sustained serious injuries three times in the past a month and half at the hand of BOY'S NAME.  Please see the attached injury report and photographs of the October 20, 2017 injury. 

After each incident, neither the teacher [NAME] nor the principal [NAME] notified me of the cause of and the circumstances around the injury.  Further, I was still not offered an explanation during my meeting with PRINCIPAL'S NAME on October 25, 2017.  More alarmingly, no action is being taken to protect YOUR DAUGHTER'S FIRST NAME from further injuries.  

The recent injuries have scarred YDFN emotionally.  Given the unresponsiveness of the school, I am forced to request again a transfer to SCHOOL NAME 2.  SCHOOL NAME 1 has failed roundly to ensure a safe environment of learning for YDFN.  I look forward to hearing from you before it is too late.
