As long as you have LEGAL DOCUMENT(s) to show that your kid has changed the name from A to B, you should not worry about it too much. If you want, you can have all the ceertificates etc copied and attached to a copy of the legal document, this is be sufficient enough for anything including apply for schools etc.

I hope the kid did not change SSN? That should be the unique ID.
