My boss gave me the form with company's signature on it. My boss took me to urgent clinic. They were very friendly. They knew I didn't have medical insurance since I only worked for them less than 3 month. I never got bill. At first, clinic asked me for the signature to be sure I paid their bills. But I didn't sign it.
Did you sign any paper when you were in hospital?
