Legal Document for Name Change

USCIS sent us RFE today requesting documentary evidence of my daughter's legal name change. It was not required to file any document for name change back in 1994 when she went to kindergarten in Canada since she was too young. Therefore, we don't have any official document to to USCIS that substantiates the name change . She has been using her English name for everthing (school, passport, etc). However, her birth certificate contains her Chinese name only, while her passport and citizen card contains her English name. My question is what to do with this RFE.
