bad dentist

I had a teeth cleaning today and the dentist made one of my tooth crown fall out.
She put it back temporarily but refused to put it back permanently because it costs.
At the end I didn't pay the copay for this cleaning. The dental office said they would have the
fee collected on me later.
Also, this dental office has always been trying to get me spend more on my teeth, and has suggested me to
redo my crown a few times before. I suspect this time they made my crown fall out on purpose.

Could someone help me on these questions:
1. I am going to see an other dentist for putting back the crown. How could I ask them to pay for cost? Do I have to go through court?

2. What should I do if an agent comes to collect the money? Does he physically come to my house? Do not like this kind of unpleasure thing happens in front of my family. Should I call cop immediately in that case?
