Your CPA didn't do anything wrong.

You should go to your bank where you have your IRA account, and get them to issue you a 1099 reflecting you taking it out, amend your 2005 tax return to reflect that, put an additional note to IRS with the tax return to explain the situation, and it should be fine. That's what I did for the same situation.
