If you go to see doctor and they accept your insurance. The one you received is statement from your insurance company. That is the way for payment.

Insurance company will state clearly what you co-payment are: most time $15.00 for each visit. You don't need to pay anything else. Unless you go to see the doctor not accepting your insurance.
