Do you still have a copy of your application form which indicated you pick year 2003 as contribution year? If so. that is easy. Just fax the form to them to proof. BTW. Why do you have to wait the tax form from them. You should be able to fill your IRA in form 1040 for tax filing. What kind of tax form are you waiting for? Another way you can do is to transfer this $3000 to another firm like Ameritrade and treat it as year 2003 contribution.
