I am not familiar with small claims procedure, but based on my own work, I think the small claims court in your county/state has the jurisdiction over him even he is in another state as the cause of action arose from his use of a credit card originally mailed to your address in your state.
Once that is determined (you should go to the court to confirm), you just serve him. Remember seeing in the movies? "You are served" and that is it. You need to hire an agent for the service of process, for which you can find the releavnt at the small claims court.
The most important thing is to make sure the court has jurisdiction. If you want to save money (not asking a lawyer), just take a few hours off to go to the court to find out.
