All the information for your issue is in the 管辖你所在地的领事馆 (for exmple, if you live in GA, AL, MS, TN, TX, SC, NC, LA, the Chinese Consulate is in Houston, TX). So, just go to the website of the Chinese Consulate and follow the instruction to do it. It is very simple procedure
