a question for tax report

Form 1040 is for Federal tax report, right? somepeople said I should file 2 tax reports,one for state one for federal,is this true? if it is true . are they the same form,so i just copy another same 1040 mail to state?
a little confused, expecting for your help.
by the way, I am a permanent resident,I don't have a regular job in 2005 and my incoming for this year almost all is tip ,and I don't have W-2 form, what should I do?
